Insurance and Billing
At the time of your visit, patients need to provide our office with the correct insurance information, including your insurance card to be copied. We ask that all co-insurance and co-payments be paid at the time the service is performed.
Your insurance coverage is an agreement between you and your insurance company. Financial responsibility rests with the patient for deductibles, co-insurance and non-covered services. In addition to personal checks, we accept Visa, MasterCard, debit cards and CareCredit.

We participate in most insurance plans. Please call your insurance company or our billing office to confirm your insurance benefits prior to your scheduled appointment.
We will perform courtesy billing for you, provided you have given us all pertinent billing information. You may receive four separate bills, the facility fee, pathology, anesthesia bill and your physician’s professional fee. Your insurance will be billed for all four of these services.
